In-Home COVID-19 Vaccinations for Home Bound Residents

On March 15th the Board of Selectmen announced Rehoboth will be launching a program to offer in-home COVID-19 vaccinations for residents who cannot make it to a vaccination center. The program is an extension of the previously announced Public / Private Partnership between the Town of Rehoboth and Anawan Pharmacy.

Rehoboth Public Health Nurse, Jaime Conlon, will lead the Town’s home bound vaccination program with the support of Amy Brown. Click on the link below to access a brief survey to determine eligibility to receive an in-home vaccination. You may also call Amy Brown in the Board of Health Office at 508-252-3099, if you need assistance in completing the survey. All survey information will be referred to Jaime Conlon for review and follow up with the resident.
